Does CVS Caremark cover Wegovy?

For weight loss: Varies by planBy Iacob Pastina. We read every source on .

CVS Caremark lists Wegovy as a preferred weight-loss drug on its templates, for plans whose sponsor chose to cover weight-loss drugs. From October 1, 2026, Zepbound joins Wegovy as an additional preferred option, so you may now have a choice.

Your employer can customize or skip weight-loss coverage. Prior authorization follows your health plan's criteria.

Who decides whether your CVS Caremark plan covers Wegovy?

Three parties shape the answer, and if you have coverage through work, your employer makes the final call.

  1. 1. CVS Caremark, the pharmacy benefit manager

Caremark builds template formularies that employers, health plans, unions and government plans hire it to run.

hired by businesses, big and small, health plans, unions, and government entities CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)

  2. 2. Your employer or plan sponsor

Your employer or plan sponsor adopts the template and can change it, so a Caremark listing is a starting point, not your plan's answer.

retain discretion to customize coverage for their members CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)

  3. 3. Your health plan

Health plans such as Aetna use Caremark for pharmacy benefits and publish their own prior authorization rules on top.

Pharmacy benefits are administered by an affiliated pharmacy benefit manager, CVS Caremark. Aetna, 2026 Pharmacy Drug Guide, Standard Control Plan (pages dated October 1, 2026)

That is why two people with the same insurance card can get opposite answers: one employer bought weight-loss coverage and the other did not.

How does prior authorization for Wegovy work at CVS Caremark?

Your prescriber asks for approval under your health plan's criteria. Aetna, which uses Caremark, publishes one example.

  • Aetna's first approval, pen8 monthsfor weight loss in adults
  • Aetna's first approval, pill6 monthsfor weight loss in adults
  • Aetna's renewal weight test5% lostor you keep off the first 5% you lost

Your prescriber's request should show:

  • Your starting BMI, from before any GLP-1 medicine, with the date it was measured.
  • Your weight-related conditions, if your plan's BMI line depends on them.
  • The diet, exercise or weight program you followed, with dates.
  • At renewal, your weight now against your starting weight.

Heart-risk criteria come from your health plan. Aetna's policy covers Wegovy to cut heart risk in adults with established heart disease and no type 2 diabetes. The document says: The patient does NOT have type 2 diabetes..

How do you find out if your CVS Caremark plan covers Wegovy?

Check the drug list, then call. It takes about four steps.

  1. Log in to Caremark.com and check the drug by name. Caremark tells members to check coverage and cost sharing for a specific medicine there. (CVS Caremark, Advanced Control Formulary (October 2025))
  2. Open Plan Summary on the Plan & Benefits menu to see your own plan's design. (CVS Caremark, Advanced Control Formulary (October 2025))
  3. Ask Caremark whether your plan adopted the October 1, 2026 template change or customized it. (CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026))
  4. Ask your HR or benefits team whether your plan covers weight-loss drugs at all.

When you call, ask:

  • Does my plan cover weight-loss drugs at all, or does it exclude them?
  • Is Wegovy on my plan's drug list, and which form: the injection pen, Wegovy HD or the tablet?
  • Did my plan adopt Caremark's October 1, 2026 formulary change for Wegovy, or customize it?
  • Where does my prescriber send the request, and how long does a decision take?
  • If you deny it, will the letter say whether it is a plan exclusion or a criteria denial, and what is my appeal deadline?

Write down the name of the person you spoke with, the date and any reference number. You need them if you appeal.

What are your three ways to pay for Wegovy?

Your route depends on whether your plan covers it, whether you have Medicare, or whether you pay cash.

1. Your plan covers it

If your plan approves Wegovy, Novo Nordisk's savings card can lower what you pay at the pharmacy, by at most $100 per month off. You cannot use it with government insurance: the copay offer is for commercial insurance; people on government insurance can use the self-pay prices only by paying outside their plan.

We read the Novo Nordisk savings terms on September 4, 2026. Your copay itself depends on your plan.

2. You have Medicare

If you have Medicare Part D, the Medicare GLP-1 Bridge charges $50 per monthly supply if you qualify. It covers the Wegovy injection and the Wegovy pill. See how the Bridge works or check your eligibility.

We checked CMS's Bridge guidance on September 26, 2026. Medicare's own checker is at medicare.gov.

3. Your plan says no, so you pay cash

  • Novo Nordisk self-payfrom $199/moNovoCare self-pay: pen $199/mo for the first two 0.25 mg or 0.5 mg fills through December 31, 2026, then $349/mo at standard strengths or $399/mo for Wegovy HD; oral pill from $149/mo, dose-dependent

We checked the Novo Nordisk self-pay page on September 20, 2026. You still need a prescription.

Compounded semaglutide costs less, and it is not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness or quality before they are sold, so talk it through with a licensed clinician. Read the FDA's explanation, then compare compounded semaglutide prices.

If CVS Caremark says no, which denials can you appeal?

Three kinds of no reach you, and only some of them are worth an appeal.

  1. 1. Your plan excludes weight-loss drugs

Usually not appealable on medical grounds

Your employer or plan left weight-loss drugs out, so meeting the medical criteria does not change the answer. Ask HR whether coverage changes at your next renewal, and ask your prescriber whether another approved use for Wegovy applies to you.

Caremark only adds Zepbound for plan sponsors that choose to cover weight-loss drugs. A sponsor that opts out leaves you with no plan route. for plan sponsors who elect to provide coverage of those medications for their members CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)

  2. 2. You do not meet the criteria yet, or paperwork is missing

    Appealable

Your plan covers the drug but the request did not show the BMI, program history or other criteria. Ask for the written reason, then have your prescriber resubmit or appeal with chart notes. A denial that rests on medical judgment can go to an independent external review.

HealthCare.gov lists denials that involve medical judgment among those you can take to external review. Any denial that involves medical judgment HealthCare.gov, External review (undated web page)

  3. 3. Your plan prefers a different drug or form

Ask for an exception, or switch

Your plan covers weight-loss drugs but not this version of Wegovy, or wants you to try something else first. Your prescriber can switch you to the listed form or ask for a formulary exception with a medical reason.

Caremark's Advanced Control Formulary file, dated October 2025, still lists Zepbound as excluded with Wegovy as a preferred option. It predates the October 1, 2026 change, so ask which version your plan follows. PREFERRED OPTIONS FOR EXCLUDED MEDICATIONS CVS Caremark, Advanced Control Formulary (October 2025)

For a health plan sold under federal rules, you file an internal appeal within 180 days of the denial notice. Your denial letter states the deadline that applies to your own plan.
