Does your insurance cover Zepbound or Wegovy? Answers by insurer
Your insurance covers Zepbound or Wegovy for weight loss only if your plan includes weight-loss drugs. At Aetna, Cigna, UnitedHealthcare, CVS Caremark and the Blue companies we read, your employer or plan sponsor makes that choice.
Each insurer still lists the drugs and publishes prior authorization rules, and in 2026 the form you are prescribed matters too: pen, KwikPen, vial or tablet.

Why can two people with the same insurer get different answers?
Two separate decisions have to line up: the drug list and your employer's benefit.
The insurer or its pharmacy benefit manager decides which drugs and forms go on the list, and the prior authorization rules. Your employer or plan sponsor then decides whether your plan covers weight-loss drugs at all. Aetna tells employers they can customize your benefits to include or exclude GLP-1 drug coverage for weight management
, and UnitedHealthcare calls its weight-loss program an optional program

What changed for Zepbound and Wegovy coverage in 2026?
Four changes matter if you are checking your plan this year.
- CVS Caremark added Zepbound back to its commercial formularies on October 1, 2026, as an additional preferred option next to Wegovy. CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)
- Express Scripts excludes the Zepbound KwikPen and vials on its 2026 and 2027 national lists, and keeps the single-dose pens among its preferred options. Express Scripts, 2026 National Preferred Formulary Exclusions (dated October 1, 2026)
- UnitedHealthcare's weight-loss program took effect in its current version on October 1, 2026, and says Wegovy tablets are typically excluded. UnitedHealthcare, Prior Authorization/Notification: Weight Loss/Appetite Suppression Medication Coverage (2026 P 1114-23) (effective October 1, 2026)
- Blue Cross Blue Shield of Massachusetts ended GLP-1 coverage for weight loss at renewals from January 1, 2026. Blue Cross Blue Shield of Massachusetts, We've changed our GLP-1 coverage (change effective at renewals from January 1, 2026)
Which insurance denials can you appeal?
You can appeal a criteria denial. A plan exclusion is usually final.
- Your plan excludes weight-loss drugs. Meeting the medical criteria does not change it. Blue Cross Blue Shield of Massachusetts writes:
This is a benefit exclusion that can't be appealed.
- You do not meet the criteria yet, or paperwork is missing. Your prescriber resubmits or appeals with chart notes, and a denial that rests on medical judgment can go to external review.
- Your plan prefers another drug or form. Your prescriber switches you to the listed form or asks for a formulary exception.

How do you check your own plan?
Search your plan's drug list, then call the number on your member ID card with five questions.
Each insurer answer above lists the exact lookup steps and the questions for that insurer. For any other plan type, including Medicaid, the Marketplace and TRICARE, our insurance lookup tool builds the questions for you, with a Zepbound prior authorization checklist and a checklist for when covered refills stop.
On Medicare, start with the Medicare GLP-1 Bridge. If your plan says no, compare Zepbound without insurance and Wegovy prices.
