What is semaglutide?
Semaglutide is a GLP-1 receptor agonist: a once-weekly injectable (or, in some formulations, a daily tablet) that mimics a gut hormone released after eating, slowing stomach emptying and reducing appetite. Novo Nordisk sells it under three brand names, and the difference between them is the approved indication rather than the molecule. Wegovy is approved for chronic weight management, Ozempic for type 2 diabetes, and Rybelsus is the oral tablet for type 2 diabetes.
Compounded semaglutide is the same molecule prepared by a compounding pharmacy rather than Novo Nordisk. It is not an FDA-approved finished product: the agency does not review it for safety, effectiveness or quality. That regulatory difference, not a difference in the molecule, is what the price gap on this page reflects. The other tirzepatide-based drugs, Zepbound and Mounjaro, are a different molecule.

The FDA has proposed permanently removing semaglutide (with tirzepatide and liraglutide) from the 503B bulk compounding list (published May 1, 2026). The public comment period closed July 30, 2026, extended from June 29 via Federal Register 2026-12937.
