Who decides whether your CVS Caremark plan covers Zepbound?
Three parties shape the answer, and if you have coverage through work, your employer makes the final call.
1. CVS Caremark, the pharmacy benefit manager
Caremark builds template formularies that employers, health plans, unions and government plans hire it to run.
hired by businesses, big and small, health plans, unions, and government entities
CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)2. Your employer or plan sponsor
Your employer or plan sponsor adopts the template and can change it, so a Caremark listing is a starting point, not your plan's answer.
retain discretion to customize coverage for their members
CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)3. Your health plan
Health plans such as Aetna use Caremark for pharmacy benefits and publish their own prior authorization rules on top.

That is why two people with the same insurance card can get opposite answers: one employer bought weight-loss coverage and the other did not.
How do you find out if your CVS Caremark plan covers Zepbound?
Check the drug list, then call. It takes about four steps.
- Log in to Caremark.com and check the drug by name. Caremark tells members to check coverage and cost sharing for a specific medicine there. (CVS Caremark, Advanced Control Formulary (October 2025))
- Open Plan Summary on the Plan & Benefits menu to see your own plan's design. (CVS Caremark, Advanced Control Formulary (October 2025))
- Ask Caremark whether your plan adopted the October 1, 2026 template change or customized it. (CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026))
- Ask your HR or benefits team whether your plan covers weight-loss drugs at all.
When you call, ask:
- Does my plan cover weight-loss drugs at all, or does it exclude them?
- Is Zepbound on my plan's drug list, and which form: the single-dose pen, the KwikPen or the vial?
- Did my plan adopt Caremark's October 1, 2026 formulary change for Zepbound, or customize it?
- Where does my prescriber send the request, and how long does a decision take?
- If you deny it, will the letter say whether it is a plan exclusion or a criteria denial, and what is my appeal deadline?
Write down the name of the person you spoke with, the date and any reference number. You need them if you appeal.

What are your three ways to pay for Zepbound?
Your route depends on whether your plan covers it, whether you have Medicare, or whether you pay cash.
1. Your plan covers it
If your plan approves Zepbound, Eli Lilly's savings card can lower what you pay at the pharmacy, by at most $100 off a one-month fill. You cannot use it with government insurance: anyone enrolled in a state, federal, or government funded healthcare program, including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, and TRICARE, plus plans routed through alternate funding programs.
We read the Eli Lilly savings terms on August 27, 2026. Your copay itself depends on your plan.
2. You have Medicare
If you have Medicare Part D, the Medicare GLP-1 Bridge charges $50 per monthly supply if you qualify. For Zepbound, the Bridge covers the KwikPen, not vials or single-dose pens. See how the Bridge works or check your eligibility.
We checked CMS's Bridge guidance on September 26, 2026. Medicare's own checker is at medicare.gov.
3. Your plan says no, so you pay cash
- Eli Lilly self-payfrom $299/moLillyDirect self-pay vials or KwikPen, from $299 per 28-day supply at the 2.5 mg starter dose, $399 at 5 mg, and $449 at 7.5 mg and above when refilled within 45 days
We checked the Eli Lilly self-pay page on August 7, 2026. You still need a prescription.

Compounded tirzepatide costs less, and it is not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness or quality before they are sold, so talk it through with a licensed clinician. Read the FDA's explanation, then compare compounded tirzepatide prices.
If CVS Caremark says no, which denials can you appeal?
Three kinds of no reach you, and only some of them are worth an appeal.
1. Your plan excludes weight-loss drugs
Usually not appealable on medical grounds
Your employer or plan left weight-loss drugs out, so meeting the medical criteria does not change the answer. Ask HR whether coverage changes at your next renewal, and ask your prescriber whether another approved use for Zepbound applies to you.
Caremark only adds Zepbound for plan sponsors that choose to cover weight-loss drugs. A sponsor that opts out leaves you with no plan route.
for plan sponsors who elect to provide coverage of those medications for their members
CVS Health, CVS Caremark delivers affordability and access to GLP-1 weight management medications with expanded coverage options (May 28, 2026)2. You do not meet the criteria yet, or paperwork is missing
Appealable
Your plan covers the drug but the request did not show the BMI, program history or other criteria. Ask for the written reason, then have your prescriber resubmit or appeal with chart notes. A denial that rests on medical judgment can go to an independent external review.
HealthCare.gov lists denials that involve medical judgment among those you can take to external review.
Any denial that involves medical judgment

Ask for an exception, or switch
Your plan covers weight-loss drugs but not this version of Zepbound, or wants you to try something else first. Your prescriber can switch you to the listed form or ask for a formulary exception with a medical reason.
Caremark's Advanced Control Formulary file, dated October 2025, still lists Zepbound as excluded with Wegovy as a preferred option. It predates the October 1, 2026 change, so ask which version your plan follows.

For a health plan sold under federal rules, you file an internal appeal within 180 days of the denial notice. Your denial letter states the deadline that applies to your own plan.
