Tirzepatide vs Semaglutide: More Weight Loss, Slightly Higher Cost
Tirzepatide (Zepbound/Mounjaro) is a dual GIP + GLP-1 receptor agonist. It activates two hormonal pathways instead of one.
In the head-to-head SURMOUNT-5 trial, tirzepatide produced 20.2% weight loss versus semaglutide's 13.7% (p<0.001).
The tradeoff: tirzepatide programs tend to cost $50-150/mo more than semaglutide-only options.

As of July 1, 2026, Medicare Part D now covers brand-name Zepbound at a $50/month copay through the GLP-1 Bridge Demonstration program. This does not apply to compounded tirzepatide, only FDA-approved Zepbound qualifies.
If you have Medicare, check whether the $50 brand copay beats the compounded options listed below before enrolling with a telehealth provider. See our Medicare GLP-1 coverage guide for eligibility requirements and how to enroll.
On May 1, 2026, the FDA proposed excluding tirzepatide from the 503B bulk compounding list. The public comment period closed July 30, 2026, extended from June 29 via Federal Register 2026-12937.
If finalized, 503B outsourcing facilities cannot compound tirzepatide in bulk. 503A patient-specific compounding from licensed pharmacies, the model used by providers on this page, remains available under narrower clinical need criteria.
After the July 30 deadline, the FDA will review public comments. A final rule is expected within 60-90 days, after which 503B facilities will have a short compliance window to wind down bulk tirzepatide production.
See our compounded tirzepatide guide and FDA 503B ban explainer for the full legal breakdown.
