Who decides whether your Aetna plan covers Zepbound?
Three parties shape the answer, and if you have coverage through work, your employer makes the final call.
1. Your pharmacy benefit manager
CVS Caremark runs Aetna's pharmacy benefit, so Aetna's drug list follows Caremark's formulary decisions.
Pharmacy benefits are administered by an affiliated pharmacy benefit manager, CVS Caremark.
Aetna, 2026 Pharmacy Drug Guide, Standard Control Plan (pages dated October 1, 2026)2. Your employer
Your employer chooses whether your plan includes weight-loss GLP-1s at all. Aetna says so in its pitch to employers.

When your plan excludes weight-loss drugs, Aetna denies the claim on that exclusion, even if you meet the medical criteria.
claims for weight loss drugs will be denied based on this exclusion

That is why two people with the same insurance card can get opposite answers: one employer bought weight-loss coverage and the other did not.
How do you find out if your Aetna plan covers Zepbound?
Check the drug list, then call. It takes about four steps.
- Log in to your Aetna member website and search the drug by name. Aetna says you can search for and estimate the cost of your drugs there. (Aetna, Formulary Updates for Standard Control Formulary (effective October 1, 2026))
- Check which form your plan lists. Aetna's drug guide lists the pen (auto-injector), and Aetna's weight-loss policy leaves Zepbound vials and the KwikPen to LillyDirect. (Aetna, Weight Loss (BMI 35) GIP-GLP-1, GLP-1 Agonists PA with Limit, policy 6450-C, version 8 (file updated April 7, 2026))
- Call the toll-free number on your member ID card and ask the questions below. (Aetna, 2026 Pharmacy Drug Guide, Standard Control Plan (pages dated October 1, 2026))
- Ask your HR or benefits team whether your employer added weight-loss drug coverage, and from which date.
When you call, ask:
- Does my plan cover weight-loss drugs at all, or does it exclude them?
- Is Zepbound on my plan's drug list, and which form: the single-dose pen, the KwikPen or the vial?
- Does my plan use Aetna's BMI 35 weight-loss policy or a different version, and what does it ask for?
- Where does my prescriber send the request, and how long does a decision take?
- If you deny it, will the letter say whether it is a plan exclusion or a criteria denial, and what is my appeal deadline?
Write down the name of the person you spoke with, the date and any reference number. You need them if you appeal.

What are your three ways to pay for Zepbound?
Your route depends on whether your plan covers it, whether you have Medicare, or whether you pay cash.
1. Your plan covers it
If your plan approves Zepbound, Eli Lilly's savings card can lower what you pay at the pharmacy, by at most $100 off a one-month fill. You cannot use it with government insurance: anyone enrolled in a state, federal, or government funded healthcare program, including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, and TRICARE, plus plans routed through alternate funding programs.
We read the Eli Lilly savings terms on August 27, 2026. Your copay itself depends on your plan.
2. You have Medicare
If you have Medicare Part D, the Medicare GLP-1 Bridge charges $50 per monthly supply if you qualify. For Zepbound, the Bridge covers the KwikPen, not vials or single-dose pens. See how the Bridge works or check your eligibility.
We checked CMS's Bridge guidance on September 26, 2026. Medicare's own checker is at medicare.gov.
3. Your plan says no, so you pay cash
- Eli Lilly self-payfrom $299/moLillyDirect self-pay vials or KwikPen, from $299 per 28-day supply at the 2.5 mg starter dose, $399 at 5 mg, and $449 at 7.5 mg and above when refilled within 45 days
We checked the Eli Lilly self-pay page on August 7, 2026. You still need a prescription.

If Aetna says no, which denials can you appeal?
Three kinds of no reach you, and only some of them are worth an appeal.
1. Your plan excludes weight-loss drugs
Usually not appealable on medical grounds
Your employer or plan left weight-loss drugs out, so meeting the medical criteria does not change the answer. Ask HR whether coverage changes at your next renewal, and ask your prescriber whether another approved use for Zepbound applies to you.
Aetna writes that many of its plans exclude weight-loss drugs, and its medical criteria do not override that exclusion.
The medical necessity criteria set forth below do not apply to health plans that specifically exclude
Aetna, Clinical Policy Bulletin 0039: Weight Reduction Programs and Devices (last reviewed March 10, 2026)2. You do not meet the criteria yet, or paperwork is missing
Appealable
Your plan covers the drug but the request did not show the BMI, program history or other criteria. Ask for the written reason, then have your prescriber resubmit or appeal with chart notes. A denial that rests on medical judgment can go to an independent external review.
HealthCare.gov lists denials that involve medical judgment among those you can take to external review.
Any denial that involves medical judgment
HealthCare.gov, External review (undated web page)3. Your plan prefers a different drug or form
Ask for an exception, or switch
Your plan covers weight-loss drugs but not this version of Zepbound, or wants you to try something else first. Your prescriber can switch you to the listed form or ask for a formulary exception with a medical reason.
Aetna's weight-loss policy leaves out Zepbound vials and the KwikPen and points you to Lilly's own program for those forms.
except Zepbound vials and Zepbound Kwikpens, which are covered under the LillyDirect manufacturer program
Aetna, Weight Loss (BMI 35) GIP-GLP-1, GLP-1 Agonists PA with Limit, policy 6450-C, version 8 (file updated April 7, 2026)
For a health plan sold under federal rules, you file an internal appeal within 180 days of the denial notice. Your denial letter states the deadline that applies to your own plan.
