Does UnitedHealthcare cover Zepbound for weight loss?
For weight loss: Varies by planBy Iacob Pastina. We read every source on .
UnitedHealthcare covers Zepbound for weight loss only if your employer chose its optional weight-loss drug program. On those plans you need prior authorization, a BMI of 30, or 27 with a weight-related condition, and lifestyle changes alongside the drug.
Optum Rx handles the prior authorization. UnitedHealthcare's general drug list does not show Zepbound at all, because the program sits outside it.

Who decides whether your UnitedHealthcare plan covers Zepbound?
Three parties shape the answer, and if you have coverage through work, your employer makes the final call.
1. Your pharmacy benefit manager
Optum Rx handles UnitedHealthcare's commercial prior authorizations. UnitedHealthcare's drug list sends prescribers to the Optum Rx team.

Benefit coverage is decided by the member's pharmacy benefit plan.

That is why two people with the same insurance card can get opposite answers: one employer bought weight-loss coverage and the other did not.
How do you find out if your UnitedHealthcare plan covers Zepbound?
Check the drug list, then call. It takes about four steps.
- Log in to your UnitedHealthcare member website and open the medication pricing and coverage tool, or call the member number on your ID card. UnitedHealthcare points members to both. (UnitedHealthcare, 2026 Prescription Drug List (effective September 1, 2026))
- Ask whether your plan includes the Weight Loss/Appetite Suppression program, the optional program that covers these drugs. (UnitedHealthcare, Prior Authorization/Notification: Weight Loss/Appetite Suppression Medication Coverage (2026 P 1114-23) (effective October 1, 2026))
- If you live in California, New Mexico, New York or North Dakota, ask whether the separate state regulatory weight-loss program applies to your plan. (UnitedHealthcare, Pharmacy Update: Notice of Changes to Prior Authorization Requirements and Coverage Criteria (bulletin dated October 1, 2026))
- Ask your HR or benefits team whether your employer elected weight-loss drug coverage.
When you call, ask:
- Does my plan cover weight-loss drugs at all, or does it exclude them?
- Is Zepbound on my plan's drug list, and which form: the single-dose pen, the KwikPen or the vial?
- Does my plan include the Weight Loss/Appetite Suppression program, and does a state regulatory program apply to me?
- Where does my prescriber send the request, and how long does a decision take?
- If you deny it, will the letter say whether it is a plan exclusion or a criteria denial, and what is my appeal deadline?
Write down the name of the person you spoke with, the date and any reference number. You need them if you appeal.

What are your three ways to pay for Zepbound?
Your route depends on whether your plan covers it, whether you have Medicare, or whether you pay cash.
1. Your plan covers it
If your plan approves Zepbound, Eli Lilly's savings card can lower what you pay at the pharmacy, by at most $100 off a one-month fill. You cannot use it with government insurance: anyone enrolled in a state, federal, or government funded healthcare program, including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, and TRICARE, plus plans routed through alternate funding programs.
We read the Eli Lilly savings terms on August 27, 2026. Your copay itself depends on your plan.
2. You have Medicare
If you have Medicare Part D, the Medicare GLP-1 Bridge charges $50 per monthly supply if you qualify. For Zepbound, the Bridge covers the KwikPen, not vials or single-dose pens. See how the Bridge works or check your eligibility.
We checked CMS's Bridge guidance on September 26, 2026. Medicare's own checker is at medicare.gov.
3. Your plan says no, so you pay cash
- Eli Lilly self-payfrom $299/moLillyDirect self-pay vials or KwikPen, from $299 per 28-day supply at the 2.5 mg starter dose, $399 at 5 mg, and $449 at 7.5 mg and above when refilled within 45 days
We checked the Eli Lilly self-pay page on August 7, 2026. You still need a prescription.

If UnitedHealthcare says no, which denials can you appeal?
Three kinds of no reach you, and only some of them are worth an appeal.
1. Your plan excludes weight-loss drugs
Usually not appealable on medical grounds
Your employer or plan left weight-loss drugs out, so meeting the medical criteria does not change the answer. Ask HR whether coverage changes at your next renewal, and ask your prescriber whether another approved use for Zepbound applies to you.
UnitedHealthcare's weight-loss program exists only where an employer elected it. Without that election your plan has no weight-loss route for these drugs.
This is an optional program
UnitedHealthcare, Prior Authorization/Notification: Weight Loss/Appetite Suppression Medication Coverage (2026 P 1114-23) (effective October 1, 2026)2. You do not meet the criteria yet, or paperwork is missing
Appealable
Your plan covers the drug but the request did not show the BMI, program history or other criteria. Ask for the written reason, then have your prescriber resubmit or appeal with chart notes. A denial that rests on medical judgment can go to an independent external review.
HealthCare.gov lists denials that involve medical judgment among those you can take to external review.
Any denial that involves medical judgment

Ask for an exception, or switch
Your plan covers weight-loss drugs but not this version of Zepbound, or wants you to try something else first. Your prescriber can switch you to the listed form or ask for a formulary exception with a medical reason.
