Does Cigna cover Zepbound for weight loss?
For weight loss: Varies by planBy Iacob Pastina. We read every source on .
Cigna covers Zepbound for weight loss only if your plan includes weight-loss drugs, and only after prior authorization. Express Scripts, Cigna's pharmacy benefit manager, prefers the single-dose Zepbound pens and excludes the KwikPen and vials on its 2026 national list.
Cigna's standard policy asks for a BMI of 30, or 27 with a weight-related condition, and three months of diet and behavior changes first.

Who decides whether your Cigna plan covers Zepbound?
Three parties shape the answer, and if you have coverage through work, your employer makes the final call.
1. Your pharmacy benefit manager
Express Scripts belongs to Evernorth, the Cigna Group's health services arm, and runs prescription plans for employers and health plans.

Express Scripts says a drug on its list can still be missing from your plan, because your employer or plan sponsor picks the benefit.

That is why two people with the same insurance card can get opposite answers: one employer bought weight-loss coverage and the other did not.
How do you find out if your Cigna plan covers Zepbound?
Check the drug list, then call. It takes about four steps.
- Log in at express-scripts.com/covered to see whether your plan covers the drug and to compare prices. Express Scripts names that page on its formulary documents. (Express Scripts, 2026 National Preferred Formulary Exclusions (dated October 1, 2026))
- Call the number on your prescription ID card and ask the questions below. (Express Scripts, 2026 National Preferred Formulary Exclusions (dated October 1, 2026))
- If your prescriber needs to ask for an exception, the request goes through the Express Scripts portal at esrx.com/PA. (Express Scripts, 2026 National Preferred Formulary Exclusions (dated October 1, 2026))
- Ask your HR or benefits team whether your plan covers weight-loss drugs, and whether it uses Cigna's standard policy or an EncircleRx version.
When you call, ask:
- Does my plan cover weight-loss drugs at all, or does it exclude them?
- Is Zepbound on my plan's drug list, and which form: the single-dose pen, the KwikPen or the vial?
- Does my plan use Cigna's standard Zepbound policy or an EncircleRx version with a BMI 32 or 35 line?
- Where does my prescriber send the request, and how long does a decision take?
- If you deny it, will the letter say whether it is a plan exclusion or a criteria denial, and what is my appeal deadline?
Write down the name of the person you spoke with, the date and any reference number. You need them if you appeal.

What are your three ways to pay for Zepbound?
Your route depends on whether your plan covers it, whether you have Medicare, or whether you pay cash.
1. Your plan covers it
If your plan approves Zepbound, Eli Lilly's savings card can lower what you pay at the pharmacy, by at most $100 off a one-month fill. You cannot use it with government insurance: anyone enrolled in a state, federal, or government funded healthcare program, including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, and TRICARE, plus plans routed through alternate funding programs.
We read the Eli Lilly savings terms on August 27, 2026. Your copay itself depends on your plan.
2. You have Medicare
If you have Medicare Part D, the Medicare GLP-1 Bridge charges $50 per monthly supply if you qualify. For Zepbound, the Bridge covers the KwikPen, not vials or single-dose pens. See how the Bridge works or check your eligibility.
We checked CMS's Bridge guidance on September 26, 2026. Medicare's own checker is at medicare.gov.
3. Your plan says no, so you pay cash
- Eli Lilly self-payfrom $299/moLillyDirect self-pay vials or KwikPen, from $299 per 28-day supply at the 2.5 mg starter dose, $399 at 5 mg, and $449 at 7.5 mg and above when refilled within 45 days
We checked the Eli Lilly self-pay page on August 7, 2026. You still need a prescription.

If Cigna says no, which denials can you appeal?
Three kinds of no reach you, and only some of them are worth an appeal.
1. Your plan excludes weight-loss drugs
Usually not appealable on medical grounds
Your employer or plan left weight-loss drugs out, so meeting the medical criteria does not change the answer. Ask HR whether coverage changes at your next renewal, and ask your prescriber whether another approved use for Zepbound applies to you.
Cigna publishes EncircleRx policies that override a plan's weight-loss exclusion only above stricter BMI lines, and Cigna writes that those lines are not clinical.
The BMI thresholds for the weight loss indications in adults are not based on clinical data
Cigna, Zepbound Benefit Exclusion Overrides Policy for EncircleRx BMI 35 (CNF1012) (review date June 17, 2026)2. You do not meet the criteria yet, or paperwork is missing
Appealable
Your plan covers the drug but the request did not show the BMI, program history or other criteria. Ask for the written reason, then have your prescriber resubmit or appeal with chart notes. A denial that rests on medical judgment can go to an independent external review.
HealthCare.gov lists denials that involve medical judgment among those you can take to external review.
Any denial that involves medical judgment

Ask for an exception, or switch
Your plan covers weight-loss drugs but not this version of Zepbound, or wants you to try something else first. Your prescriber can switch you to the listed form or ask for a formulary exception with a medical reason.
Express Scripts' 2026 National Preferred Formulary excludes the Zepbound KwikPen and vials and names the single-dose Zepbound pens among the preferred alternatives.
