Louisiana Medicaid covers Wegovy for heart risk or MASH and Zepbound for sleep apnea, not GLP-1 medicines for weight loss alone, under its preferred drug list effective July 1, 2026 and updated October 1, 2026. Louisiana Medicaid can approve Wegovy injection or tablets if you are 45 or older with a prior heart attack, stroke or artery disease and no diabetes, and it offers a Wegovy injection route if you have MASH liver disease with F2 or F3 fibrosis. It can approve Zepbound pens if you are an adult with moderate to severe sleep apnea and a BMI of at least 30. To renew, Louisiana Medicaid looks for at least 5% weight loss. These rules apply in fee-for-service and every Medicaid managed care plan. Louisiana Medicaid covers orlistat (Xenical), a non-GLP-1 pill, from age 12 if you have a BMI of 27 or more and a qualifying diagnosis. The Legislature passed Senate Bill 433 in 2026, which would require coverage of FDA-approved weight-loss medicines only after lawmakers fund it, so ask LDH whether that coverage has started.
A drug on the preferred list is not covered for every use, so check the rule for yours. A weight-loss exclusion leaves diabetes and other medical requests to their own rules.

What should you ask Louisiana Medicaid before you pay?

An “insurance accepted” badge does not tell you your benefit, so ask your plan how its review works.
What paperwork does prior authorization need?
Your prescriber sends Louisiana Medicaid a request with the records that show you meet the rule for your drug and diagnosis, and Louisiana Medicaid decides.

What your prescriber usually sends:
- The condition the request relies on, as Louisiana Medicaid names it: Wegovy for heart risk, as injection or tablets, or MASH, as the injection; Zepbound for sleep apnea; Orlistat for weight loss from age 12, with a BMI of 27 or more and a qualifying diagnosis; an older pill, not a GLP-1.
- Your BMI, with the height and weight it comes from and the date your prescriber measured them.
- Chart notes that document the condition, such as a sleep study for sleep apnea or the liver scarring stage for MASH, where the rule asks for them.
- The exact drug, form and dose your prescriber requests, and whether you have diabetes, because some routes exclude it.
- What you have tried already: diet and activity changes, and any earlier medicine the rule asks about.
- For a renewal, your weight, refill and treatment records since the first approval.
Your prescriber works from these Louisiana Medicaid documents:
- Louisiana Medicaid Wegovy clinical criteria: cardiovascular (injection or tablet) and MASH (injection only)

How does Medicare cover GLP-1s in Louisiana?
