Its pharmacy manual effective April 2026 excludes anorexiant products for all Medicaid plans, and its drug list offers no weight-management approval route. Wyoming Medicaid can approve Wegovy if you have a BMI of 27 or higher and have had a heart attack or stroke or have peripheral artery disease. It also lists Wegovy injections for MASH, a liver disease, with prior authorization. It can approve Zepbound if you are an adult with obesity and moderate to severe obstructive sleep apnea shown on a sleep study in the past 12 months, and to continue, you must show at least 5% weight loss at six months. Wyoming Medicaid handles diabetes GLP-1 medicines in a separate class and reviews every request.

A drug on the preferred list is not covered for every use, so check the rule for yours. A weight-loss exclusion leaves diabetes and other medical requests to their own rules.

  • Whether your plan covers your exact drug, form and dose for your diagnosis and age.
  • Whether you are in fee-for-service or a managed-care plan, and who handles your prior authorization.
  • Which records your prescriber must send with the request.
  • Which pharmacy can fill it, and your drug copay apart from any visit, lab or membership fees.
  • Whether paying cash affects your benefits.
  • If the plan says no, the reason in writing, how to ask for a review and the deadline on the notice.

An “insurance accepted” badge does not tell you your benefit, so ask your plan how its review works.

What paperwork does prior authorization need?

Your prescriber sends Wyoming Medicaid a request with the records that show you meet the rule for your drug and diagnosis, and Wyoming Medicaid decides.

What Wyoming Medicaid's own criteria ask for, from the documents we read:

  • For Zepbound, a sleep study in the past 12 months showing moderate to severe obstructive sleep apnea; to continue, at least 5% weight loss at six months.

What your prescriber usually sends:

  • The condition the request relies on, as Wyoming Medicaid names it: Wegovy for heart risk, or MASH as injections; Zepbound for sleep apnea.
  • Your BMI, with the height and weight it comes from and the date your prescriber measured them.
  • Chart notes that document the condition, such as a sleep study for sleep apnea or the liver scarring stage for MASH, where the rule asks for them.
  • The exact drug, form and dose your prescriber requests, and whether you have diabetes, because some routes exclude it.
  • What you have tried already: diet and activity changes, and any earlier medicine the rule asks about.
  • For a renewal, your weight, refill and treatment records since the first approval.

How does Medicare cover GLP-1s in Wyoming?

Medicare's GLP-1 Bridge follows federal rules in every state, so Wyoming Medicaid's policy does not decide your Medicare benefit.
