You need to be in a Part D plan or an MA-PD, your prescriber files the prior authorization with CMS rather than with your plan, and you need a BMI of 35 or higher, or 27 or higher with prediabetes, a prior heart attack, a prior stroke or symptomatic peripheral artery disease.
