Zepbound dosage: the full schedule, in every pen and vial
Every Zepbound patient spends their first month on a dose the label refuses to call a treatment dose. The schedule itself is short: start at 2.5 mg once weekly for 4 weeks, move to 5 mg, then increase in 2.5 mg increments with at least 4 weeks on each dose. Maintenance for weight reduction is 5 mg, 10 mg, or 15 mg once weekly, and 15 mg is the maximum for every indication. What most dosage pages skip is that the same six doses now arrive in four different containers, a single-dose pen, a single-dose vial, a multi-dose vial, and the KwikPen, and the volume you inject changes between them. The weeks column is the fastest path the label permits, 4 weeks per dose. The label ties the slow escalation to reducing gastrointestinal side effects, and tells prescribers to weigh response and tolerability when picking a maintenance dose; if a maintenance dose is not tolerated, its answer is a lower maintenance dose. 2.5 mg is a starter, not a treatment dose
The label's own words: the 2.5 mg dosage "is for treatment initiation and is not approved as a maintenance dosage." Four weeks is the entire intended life of that dose. It exists to let your gut adapt before the drug reaches strengths that produced the published trial results, which is why a program that keeps you at 2.5 mg to stretch a budget is selling you the on-ramp forever.

The label lists four presentations. All six dose strengths exist in each, and the multi-dose formats hold four weekly doses per container.

Storage differs too: a single-dose pen or vial survives at most 21 days out of the refrigerator, while an opened multi-dose vial or KwikPen must be discarded 30 days after first use, or after its 4 doses, even with medicine left inside.

Where dosing errors actually happen: the units trap
Brand Zepbound in a pen involves no arithmetic at all. The moment a syringe enters the picture, the numbers stop transferring between containers: the same 5 mg dose is 0.5 mL from a single-dose vial and 0.6 mL from a multi-dose vial, because the concentrations differ. Copying a volume from one format to another, or from a friend's prescription, is the exact failure mode the label's per-presentation training requirement exists to prevent.

The manufacturer's ladder, exactly as Lilly publishes it: LillyDirect self-pay vials, from $299/mo at the 2.5 mg starter dose, $399 at 5 mg, and $449 at 7.5 mg and above when refilled within 45 days.
